Referral Program
Admission / Referral Process:
During the admission process the Intake Facilitator provides necessary support to applicants and their support network. The Intake Facilitator provides case management services and assists with making appropriate referrals in the area.
To determine if STEP UP! ABI Recovery can provide the appropriate service, each application is reviewed on an individual basis.
Criteria:
- Have experienced an acquired brain injury
- Thirteen years or older
- Reside in the City of Hamilton or surrounding area
