Application Process for Services
You may apply directly to the Step Up! ABI Recovery division by contacting:

To receive service, you must:
- Have experienced an acquired brain injury
- Be 16 years or older (for all services)
- Be at least 13 years old for adolescent services. (Currently no residential services are offered)
Application Form For Services
The application form is available in Adobe PDF file format. If you do not have Acrobat Reader installed on your computer, you will need to download it from Adobe. An application form can be mailed upon request.
To determine what programming Step Up! ABI Recovery can provide, each application is reviewed on an individual basis.
